Build intake forms with a canvas, field types, live preview, and JSON export.

Form Builder is a browser tool for assembling intake forms without a login wall. Drag in text, choice, and date fields, edit labels and validation, then preview the form as a respondent would see it. The layout is stored in the browser so you can keep working and export a JSON definition when you are ready.
